• Oracle Account Number. This can be found in the account details inside Salesforce.
  
  To ensure the activation number is recorded properly for an account/school, please make sure to include the Oracle Account Number in your submission.

• Name and Address of Institution (please include name of campus if applicable)
• Primary Contact (person ALEKS can contact if there are questions regarding your submission)
• Instructor(s) First and Last Names, along with Email addresses
• List of ALEKS Higher ED Course Products
• For cases involving course copy or master template link, have the 10-digit course code available
